At the bottom of this page is an image of a form for the standard rules of supervision for probation, intermediate punishment, and parole used in Adams County, PA. The same form is also used for ARD (i.e., Accelerated Rehabilitative Disposition).
Use
The form is used by the Department of Probation Services and the defendant. A probation officer or other representative will require the defendant to sign the form if the court has imposed probation, parole, or intermediate punishment as part of the sentence, or if the defendant has been admitted into the ARD program.
